HAZARDS SURVEY In June 2006 we put together a survey of visually impaired to gather information about everyday hazards "which cause the most distress or even prevent some people from leaving their home". Please feel free to take part in our short survey here on the website. There follows two word documents, the first of which is a letter to introduce you to the survey, the second is the survey form itself, for you to complete as a word document and mail back to us. |
